Text Messaging Program & Consent

Impact Witness is a documentation platform operated by Law Box LLC. A participating personal injury law firm uses it to let its own clients record the day-to-day impact of an injury by text message, so the firm can document that impact for the client's case. This page describes the text-messaging program and shows the authorization form a client signs to opt in.
Participation is optional
Enrolling in Impact Witness and consenting to receive text messages is entirely voluntary. It is never a condition of retaining the law firm, of receiving legal representation, or of completing any transaction. A client who declines to receive text messages still receives full legal services from their firm.
How a client opts in
Impact Witness is a private, client-only service. There is no public or web-based sign-up: a person cannot opt in from this website. Only an existing, signed client of a participating law firm can be enrolled, and only after that client gives prior express written consent by signing the standalone SMS authorization form below. The firm presents this form to the client in person at intake, separate from the firm's engagement agreement. The firm enters the client's mobile number into Impact Witness only after the signed form is on file, and keeps a record of that consent. No message is ever sent before the client opts in. This page and the form below cover SMS consent only; the privilege and work-product terms for the service are addressed separately, in the firm's standalone client agreement.
Program details
Program name: Impact Witness.
What we send: after a client opts in, the service may send an automatic delivery receipt confirming a message was received (e.g., "Message received"), a one-time welcome message, and follow-up questions and check-ins that the client's legal team reviews and sends. Substantive case-related messages are authored and sent by the firm, not automatically.
Message frequency varies and depends on how often a client chooses to message and on case-related events.
Message and data rates may apply. Standard message and data rates from the client's mobile carrier may apply.
Help: a client can reply HELP at any time, or email admin@impactwitness.com.
Opt out: a client can cancel messages at any time by replying STOP. After opting out they receive one confirmation message and no further messages unless they reply START to resume.
No marketing. The service never sends promotional, marketing, or sales text messages.

Your law firm uses Impact Witness, a secure documentation service, to record how your injury has affected your daily life. With your permission, Impact Witness will text you so you can share updates about your recovery in your own words. Please complete and sign this form before your firm enrolls you.
Authorization. I authorize my law firm named above and its documentation service, Impact Witness, to contact me by text message (SMS) at the mobile number I provided, regarding my case. This may include automated follow-up questions in response to messages I send, and periodic check-in reminders.
I understand that: message frequency varies; message and data rates may apply; I may reply STOP at any time to end text messages, or HELP for help; my consent to receive text messages is NOT a condition of my legal representation; no promotional or marketing text messages will be sent; and how my information is collected, used, and protected is described at impactwitness.com/privacy.
